Abstract
Objective To study the distribution,clinical characteristics and related influencing factors for nosocomial infection after neurosurgery.Methods According to whether nosocomial infection occurred in 212 patients after neurosurgery in the Xingtai People's Hospital from January 2022 to June 2024,they were divided into group A(31 cases,nosocomial infection occurred)and group B(181 cases,no nosocomial infection occurred).The distribution of pathogenic bacteria and drug resistance of main pathogenic bacteria in patients with nosocomial infection after neurosurgery were counted.The general data and clinical characteristics of the two groups were compared,and the related factors influencing nosocomial infection were analyzed.Results A total of 35 strains of pathogenic bacteria were isolated from 31 patients with nosocomial infection after neurosurgery,including 68.57%of Gram-negative bacteria,and 31.43%of Gram-positive bacteria.Klebsiella pneumoniae,Acinetobacter baumannii,and Staphylococcus aureus were the dominant bacteria.The resistance rates of Klebsiella pneumoniae to cefotaxime,ceftriaxone,imipenem,meropenem,ciprofloxacin,levofloxacin and polymyxin were all lower than 50.00%.The resistance rates of Staphylococcus aureus to vancomycin,linezolid,tetracycline and tigecycline were 0.00%.The age,proportion of patients using glucocorticoids,proportion of patients undergoing reoperation,proportion of patients with emergency surgery,intraoperative blood loss,and proportion of patients using ventilator in group A were significantly higher than those of group B(P<0.05).The operation time,antibiotic use time,catheter use time,central venous catheterization time,drainage tube use time,and hospitalization time were significantly longer than those of group B,and the Glasgow coma score(GCS)was significantly lower(P<0.05).The use of glucocorticoids,longer operation time,longer use of antibiotics,longer use of ventilators,longer use of catheters,and longer central venous catheterization were independent risk factors for nosocomial infection after neurosurgery(P<0.05).Conclusion The main pathogens of nosocomial infection in patients after neurosurgery consist of Gram-negative bacteria and Gram-positive bacteria.The influencing factors are the use of glucocorticoids,operation time,antibiotic use time,ventilator use,catheter use time,and central venous catheterization time.关键词
