Abstract
Objective To explore the correlation between the expression of hepatocyte nuclear factor 4α(HNF4α)and Villin with peripheral blood eosinophil(EOS)and its value for prognosis evaluation in children with eosinophilic gastroenteritis(EG).Methods A total of 60 children with EG admitted to Linfen Maternal&Child Healthcare Hospital from January 2022 to December 2023 were as the case group.During same period,and 30 children at same age with no abnormal pathological biopsy under gastroscopy and EOS<10/high-powered field(hpf),and normal peripheral blood EOS count and total immunoglobulin E determination were as the control group.Children in the case group received oral management of loratadine and famotidine tablets.The positive rates of HNF4α and Villin,and ndoscopic results were assessed.The relative expression of HNF4α mRNA and Villin mRNA,and the levels of EOS and immunoglobulin E(IgE)were compared between the case group(at acute phase and remission phase)and the control group.The correlation between the expression of HNF4α mRNA and Villin mRNA with EOS was analyzed,and the results of gastroscopy were compared between the case group and the control group.The factors influencing prognosis of EG were analyzed by multiple Logistic regression analysis.The receiver operating characteristic(ROC)curve was plotted to analyze the predictive value of HNF4α mRNA and Villin mRNA expression on the prognosis of EG.Results In the results of immunohistochemical analysis,the positive expression rates of HNF4α(20.00%[12/60]vs(43.33%[13/30])and Villin(16.67%[10/60]vs 40.00%[12/30])in the case group were significantly lower than those of the control group(χ2=5.428,5.896,P=0.020、0.015).Significantly lower HNF4α mRNA and Villin mRNA in the gastric mucosa,and higher EOS and IgE were detected of children with acute phase of EG compared to those with remission phase of EC and the subjects of control group,which in children with remission phase of EC were significantly pronounced compared with the control group(all P<0.05).There was a negative correlation between HNF4α mRNA,Villin mRNA and EOS in gastric mucosa of children with EG(r=-0.572,-0.549,P<0.000).Gastroscopy results showed that the incidences of mucosal congestion,edema,erosion,ulcer,nodular swelling or hyperplasia in the case group were significantly higher than those of the control group(P<0.05).Multiple Logistic regression analysis results proofed that HNF4α mRNA and Villin mRNA were independent influencing factors for prognosis of EG(P<0.05).The expression of HNF4α mRNA and Villin mRNA in the patients with poor prognosis was significantly lower relative to the patients with good prognosis(P<0.05).The area under the curve(AUC)of HNF4α mRNA in predicting the prognosis of EG was 0.741,and the optimal cut-off value was 1.12.The AUC of Villin mRNA in predicting the prognosis of EG was 0.680,and the optimal cut-off value was 0.71.HNF4α mRNA and Villin mRNA had certain value in the prognosis of children with EG(P<0.05).Conclusion The expressions of HNF4α mRNA and Villin mRNA in gastric mucosa of children with EG are significantly lower than those of normal tissues,and their expression are negatively correlated with EOS.The prognosis is worse when the two expressions are low.关键词
