超声引导下胸神经阻滞对乳腺癌改良根治术患者疼痛递质、血流动力学的影响
Effects of Ultrasound-Guided Thoracic Nerve Block on Pain Mediators and Hemodynamics in Patients Undergoing Modified Radical Mastectomy

Abstract
Objective To analyze the effect of ultrasound-guided thoracic nerve block on pain mediators and hemodynamics in breast cancer patients undergoing modified radical mastectomy.Methods The clinical data of 132 patients who underwent modified radical mastectomy from November 2022 to November 2023 at Qinhuangdao Maternal and Child Health Center were analyzed retrospectively,and the patients were divided into a study group(n=66)and a control group(n=66)by anesthesia methods.The operation time,intraoperative bleeding,the anesthesia rescue rate,the Hemodynamics at different time points[mean arterial pressure(MAP),heart rate(HR)],pain mediators[neuropeptide Y(NPY),prostaglandin E2(PGE2),substance P(SP)],visual analogue scale(VAS),and adverse reactions were compared between the two groups.Results The postoperative recovery time was shorter and the anesthesia rescue rate was lower in the study group compared with the control group(P<0.05).Compared with T0,MAP and HR at T1 and T2 decreased in both groups,and the magnitude of reduction was smaller in the study group compared with the control group(P<0.05).The levels of NPY,PGE2,and SP in the study and control groups were lower than those before operation 24 hours after operation,and the levels of NPY,PGE2,and SP in the study group were lower compared with the control group(P<0.05).The VAS score of the study group was lower compared with the control group at 6 hours and 24 hours after operation(P<0.05).The incidence of adverse reactions in the study group was lower compared with the control group(P<0.05).Conclusion Ultrasound-guided thoracic nerve block provides effective anesthesia for modified radical mastectomy.It shortens postoperative recovery time,reduces anesthesia rescue rates,alleviates postoperative pain,maintains hemodynamic stability,and has a high safety profile.关键词
