孙伟正运用利湿清热法治疗阵发性睡眠性血红蛋白尿症经验
Sun Weizheng's experience in the treatment of paroxysmal sleep haemoglobinuria using the method of promoting dampness and clearing heat

Abstract
Paroxysmal nocturnal hemoglobinuria(PNH)is a rare acquired haematopoietic stem cell clonal disease caused by a mutation in the PIG-A gene on somatic cells,resulting in reduced or absent expression of GPI anchor protein(GPI-AP).The main clinical manifestation is sleep-related,intermittent episodes of intravascular haemolytic haemoglobinuria,which may be accompanied by symptoms such as haemolytic anaemia,thrombosis and haematopoietic dysfunction.Professor Sun Weizheng posits that paroxysmal sleep haemoglobinuria belongs to the category of huangdan(jaundice)in TCM,and the key mechanism is attributable to dampness and heat.He proposes that dampness is more significant than heat and that treatment should therefore follow the basic principle of eliminate pathogenic factors and reinforce healthy qi with dampness as the mainstay and heat as a supplement.Professor Sun further contends that it is imperative to differentiate between the symptoms of the disease and its symptoms,the main priority of the real and the imaginary,and that both attack and tonic should be applied.Professor Sun's approach is informed by the Chinese medicinal tradition,which holds that the treatment of jaundice necessitates the facilitation of urine.Professor Sun has devised an original prescription,Yihuang Decoction(Jaundice-Removing Decoction),which employs the therapeutic modalities of"facilitating dampness and clearing heat"to address the condition.The efficacy of this approach is evident from its notable clinical outcomes.关键词
