激活素A结合振幅整合脑电图在早产儿脑损伤早期诊断中的应用
Application of activin A combined with amplitude-integrated electroencephalogram in early diagnosis of brain injury in premature infants

Abstract
Objective To investigate the early diagnostic value of serum activin A(ACT A)levels combined with amplitude-integrated electroencephalogram(aEEG)in identifying brain injury in premature infants(BIPI).Meth-ods Premature infants hospitalized between May 2020 and May 2022 were enrolled and divided into a mild brain injury group,a severe brain injury group,and a control group based on cranial ultrasound and magnetic resonance imaging(MRI)findings.Serum ACT A levels were measured within 72 hours of birth,and aEEG monitoring was performed for 4-6 hours.aEEG scores were calculated and analyzed.Results A total of 90 premature infants were included,with 40 in the mild injury group,13 in the severe injury group,and 37 in the control group.Baseline characteristics such as gen-der,gestational age,birth weight,and delivery mode showed no significant differences among the groups(P>0.05).The median aEEG scores for the mild injury,severe injury,and control groups were 10,5,and 13,respectively.Median ACT A levels were 24.30 ng/mL,30.50 ng/mL,and 19.56 ng/mL,respectively.Both the mild and severe injury groups had significantly lower aEEG scores and higher ACT A levels compared to the control group(P<0.01).In severe injury group,the aEEG scores were lower,and the ACT A levels were higher than those in the mild injury group(P<0.01).A negative correlation was found between ACT A levels and aEEG scores(r=-0.566,P<0.05).For early prediction of severe brain injury,the areas under the ROC curve(AUCs)for aEEG scores,ACT A levels,and their combination were 0.996,0.908,and 0.998,respectively,with sensitivities of 1 and specificities of 0.948,0.753,and 0.987,respective-ly.For predicting BIPI,the AUCs for aEEG scores,ACT A levels,and their combination were 0.896,0.850,and 0.935,respectively,with sensitivities of 0.943,0.868,and 0.830,and specificities of 0.730,0.811,and 0.892,re-spectively.Conclusion Both aEEG scores and ACT A levels are sensitive and specific indicators for early prediction of BIPI.Combining these two indicators provides superior predictive value compared to using either indicator alone.关键词
