海南地区2020株结核分枝杆菌耐药特征分析
Drug resistance characteristics of 2020 strains of Mycobacterium tuberculosis in Hainan Province

Abstract
Objective:To investigate the drug resistance characteristics of tuberculosis patients in Hainan in recent years,and to provide new data support for the formulation of tuberculosis prevention and control strategies in Hainan.Methods:2 020 strains of Mycobacterium tuberculosis cultured positive in sputum or alveolar lavage fluid of the Second Affiliated Hospital of Hainan Med-ical College(Hainan Tuberculosis Hospital)from January 1,2018 to December 31,2022 were collected.The sensitivity test of 8 anti-tuberculosis drugs was conducted by proportional method,and the drug resistance of 2 020 strains was analyzed.Results:The total drug resistance rate of the 2 020 strains was 36.2%(732/2 020),the single drug resistance rate was 12.0%(242/2 020),the rifampicin drug resistance rate was 23.2%(469/2 020),the multidrug resistance rate was 18.9%(383/2 020),and the wide-spread drug resistance rate was 1.3%(26/2 020).The single resistance rates of 8 drugs in the order of high to low were isoniazid(25.9%,524/2 020),rifampicin(23.2%,469/2 020),streptomycin(17.2%,348/2 020),ofloxacin(12.6%,254/2 020),ethambu-tol(9.1%,184/2 020),capreomycin(2.0%,41/2 020),kanamycin(1.9%,39/2 020),isonicotinamide protionamid(0.9%,19/2 020);The drug resistance rate of retreatment was significantly higher than that of initial treatment.Among the 732 patients with drug resistance,82.6%(605/732)were males and 17.4%(127/732)were females,but there was no significant difference in to-tal drug resistancewas the highest incidence,and the total drug resistance rate of all age groups was significantly different(χ2=10.136,P<0.05);In recent years,the annual incidence of drug-resistant tuberculosis is on the rise;The number of drug-resistant tuberculosis cases in different regions of Hainan Province was unevenly distributed,and the coastal areas were more than the cen-tral areas.Conclusion:The drug resistance of tuberculosis in Hainan is still serious.It is necessary to strengthen the detection of drug resistance,do a good job in the screening of drug resistance in key groups,strictly implement the standardized treatment and management of tuberculosis,and prevent the spread of drug resistant tuberculosis.关键词
