Effect of residual shunt on migraine and cryptogenic stroke after percutaneous patent foramen ovale closure

Abstract
Objective:To evaluate the impact of residual shunt degree after patent foramen ovale(PFO)closure on postopera-tive symptom relief and recurrence of stroke in patients with migraine and cryptogenic stroke(CS).Methods:The clinical data of 73 PFO patients with migraine and 73 PFO patients with CS who underwent percutaneous patent foramen ovale closure at the Sec-ond Affiliated Hospital of Hainan Medical College from June 2021 to February 2023 were analyzed.In the next 6 months,the de-gree of residual shunting was evaluated through transesophageal echocardiography(TEE)combined with right heart contrast echo-cardiography(ASCE),and then through the Paradoxical Embolism Scale(RoPE),Migraine Disability Assessment Question-naire(MIDAS),etc.The test questionnaires respectively assessed the relationship between stroke and abnormal emboli and the degree of migraine.Patients with different comorbidities were divided into two groups according to the size of the postoperative re-sidual shunt:no residual shunt group,mild residual shunt group,and moderate residual shunt group.and above residual shunt groups,and compare the differences in stroke induction and migraine severity among patients with different postoperative residual shunt volumes in each of the three groups.Results:100%of patients with no residual shunt or mild residual shunt combined with migraine after closure of patent foramen ovale had effective improvement of migraine symptoms,67.1%of patients had complete elimination of migraine symptoms,and all patients with cryptogenic stroke had no recurrence of stroke after closure of patent fora-men ovale and the associated cerebral infarction sequelae were improved before surgery.Conclusions:Patients with patent fora-men ovale who suffer from migraine and cryptogenic stroke can achieve treatment goals even if there is mild residual shunting after percutaneous patent foramen ovale closure.The absence of residual shunting after PFO closure may be an independent predictor of complete resolution of migraine burden.Blurred vision in aura symptoms may be a relevant factor in the occurrence of residual shunting after surgery.关键词
