Abstract
Objective:To investigate the predictive value of time in range(TIR)and glycated hemoglobin(HbA1c)on complication risk in patients with type 2 diabetes mellitus(T2DM).Methods:A total of 155 T2DM patients admitted to the department of endocrinology,Yancheng First Hospital Affiliated of Nanjing University Medical College/Yancheng No.1 People's Hospital from January 2022 to June 2023 were enrolled.Data were collected using a general information questionnaire,and blood glucose indicators were measured and analyzed.Patients were categorized into a complications group and a non-complications group based on complication occurrence.Risk factors for complications were identified,and the predictive efficacy of TIR and HbA1c was evaluated using receiver operating characteristic(ROC)curve analysis.Results:Complications were observed in 41 patients.Univariate and Logistic regression analyses revealed that age≥60 years,disease duration,triglyceride levels,serum creatinine,Diabetes Self-Management Behavior Scale(SDSCA)score,and HbA1c were significant influencing factors for complications(P<0.05).For TIR in predicting complication risk,the area under the ROC curve(AUC)was 0.872[95%CI(0.796,0.948)],with sensitivity of 0.817,specificity of 0.845,Youden index of 0.662,and an optimal cutoff value of 78.59%.For HbA1c,the AUC was 0.835[95%CI(0.750,0.919)],with sensitivity of 0.829,specificity of 0.841,Youden index of 0.670,and an optimal cutoff value of 8.81%.The combination of TIR and HbA1c achieved an AUC of 0.931[95%CI(0.880,0.982)],sensitivity of 0.841,specificity of 0.873,and Youden index of 0.714.Conclusions:Both TIR and HbA1c are significant predictors of complication risk in T2DM patients.The combination of HbA1c and TIR demonstrates superior predictive performance,serving as a critical clinical indicator for assessing complication risk in T2DM patients.关键词
