Abstract
Objective:To develop a nursing quality evaluation index system for preventing intraoperative hypothermia in orthopedic surgery patients and assess its clinical application outcomes.Methods:Based on literature review and the"structure-process-outcome"model,preliminary indicators for evaluating nursing quality in preventing intraoperative hypothermia were identified.A draft index system was developed,refined through expert selection and Delphi consultations,and finalized.Using convenience sampling,150 orthopedic surgery patients admitted to the operating room of the Affiliated Hospital of Xuzhou Medical University,Jiangsu province,were divided into a control group(July 2022—February 2023)and an observation group(March 2023—October 2023).The control group received routine nursing care,while the observation group was managed using the developed index system.Intraoperative hypothermia incidence and nursing satisfaction were compared between the groups.Results:The first-round Delphi survey achieved a 94.74%response rate,with an expert authority coefficient of 0.836 and Kendall's W coefficient of 0.438(χ²=35.825,P<0.001).The second-round survey had a 100.00%response rate,with an expert authority coefficient of 0.848 and Kendall's W coefficient of 0.492(χ²=41.273,P<0.001).The finalized system included three levels,3 first-level,10 second-level,and 34 third-level indicators,with coefficient of variation(CV)values of 0.078～0.147.The observation group exhibited a significantly lower intraoperative hypothermia incidence(2.00%vs 9.33%,P<0.05)and higher satisfaction scores in preoperative education,risk assessment,preventive nursing,and communication(P<0.05).Conclusions:Patients undergoing orthopedic surgery have the risk of intraoperative hypothermia.This indicator system is comprehensive and reliable,and can achieve effective evaluation and continuous improvement of clinical nursing quality.关键词
