CBCT在上颌第一前磨牙桩道预备中的应用

Abstract
Objective:To evaluate of root canal morphology of maxillary first premolars using cone-beam computed tomography(CBCT)and to provide data support for post space preparation.Methods:CBCT data of 150 maxillary first premo-lars were collected and divided into three groups of 50 cases each according to Vertucci's classification of root canal types:1 root with 1 canal(Type Ⅰ),1 root with 2 canals(Type Ⅱ),and 2 roots with 2 canals(Type Ⅳ).The different directions of root wall thickness and diameter were measured at six cross-sections,ranging between 4～9 mm from the apex.The proportion of cases which theoretical residual root canal wall thickness＞1 mm after preparation with different sizes of peeso reamers was calculated.Results:There was a significant difference in root length between the 1 root with 1 canal group and the 1 root with 2 canals group.In the 1 root with 1 canal group and the 2 roots with 2 canals group,the median of root wall thickness in all directions was＞1 mm at distances more than 6 mm from the apex.In the 1 root with 2 canals group,the median of root wall thickness in all directions was＞1 mm at distances more than 5 mm from the apex.In the 1 root with 1 canal group,the pro-portion of cases which theoretical residual root canal wall thickness＞1 mm after preparation with 1# peeso reamer at 7 mm from the apex exceeded 50％.In the 1 root with 2 canals group,the proportion of cases with 1# peeso reamer and partially with 2# peeso reamer at 6 mm from the apex exceeded 50％.In the 2 roots with 2 canals group,the proportion of cases with 2# pee-so reamer at 6 mm from the apex exceeded 50％.Conclusion:CBCT provides valuable guidance for post space preparation in maxillary first premolars,for Type Ⅰ root canals,1 # peeso reamer can be used to prepare up to 6～7 mm from the apex,for Type Ⅱ,1#-2# peeso reamers can be used up to 5～6 mm from the apex,and for Type Ⅳ,1#-2# peeso reamers can be used up to 6 mm from the apex.关键词
