摘要

Abstract

Objective To investigate the effects of different doses of probiotics on glucose and lipid metabolism indi-cators in patients with gestational diabetes mellitus(GDM).Methods From June 2023 to June 2024,120 patients with GDM diagnosed at 24-28 weeks of pregnancy were selected and divided into high dose group(n=40),low dose group(n=40)and placebo group(n=40)according to the random number table method.The high dose group was treated with 2 bags of probiotic preparations(containing Lactobacillus plantarum LP45,Lactobacillus acid La28,Lactobacillus paracasei,Bifidobacterium bifidobacterium L531,each bag containing 6×1010 CFU of probiotics),the low dose group was treated with 1 bag of probiotic preparations,and the placebo group was treated with microcellulose placebo.The dietary status,blood glu-cose,islet function,lipid metabolism and reasonable intake of the three groups were observed before and after intervention,and the effects on improving the adverse pregnancy outcomes of GDM patients were observed.Results After intervention,there were significant differences in total intake,carbohydrate intake and protein intake between high dose group and low dose group,as well as between high-dose group and placebo group,and there were significant differences in total intake and carbo-hydrate intake between low-dose group and placebo group(P<0.05,P<0.01).There were significant differences in fasting blood glucose and 2 h postprandial blood glucose between high dose group and low dose group,high dose group and placebo group,low dose group and placebo group(P<0.05,P<0.01).There were significant differences in homeostasis model as-sessment of insulin resistance(HOMA-IR)and homeostasis model assessment of islet β cell function(HOMA-IR)between the high dose group and the low dose group after intervention(P<0.05).There were also significant differences in fasting in-sulin(FINS),HOMA-IR and HOMA-β cell function between the high dose group and the placebo group,and between the low dose group and the placebo group after intervention(P<0.05,P<0.01).There were significant differences in the tri-glyceride level between the high dose group and the low dose group after intervention,in the triglyceride,total cholesterol,and low-density lipoprotein cholesterol levels between the high dose group and the placebo group after intervention,and in the low-density lipoprotein cholesterol level between the low dose group and the placebo group after intervention(P<0.05,P<0.01).There was a significant difference in the incidence of neonatal hypoglycemia between the high dose group and the pla-cebo group(P<0.05),but there were no significant differences in the incidence of fetal distress,macrosomia,puerperal in-fection,and postpartum hemorrhage among the three groups(P>0.05).Conclusion Probiotics can optimize the levels of blood glucose,blood lipid,and islet function in GDM patients,and high-dose probiotics have better effects.
