All-on-4种植即刻修复治疗老年牙列缺失患者的效果分析
Analysis of the Effect of Immediate Implant Placement and Restoration with All-on-4 in the Treatment of Elderly Patients with Dentition Loss

Abstract
Objective To analyze the application effect of immediate implant placement and restoration with All-on-4 in elderly patients with dentition loss.Methods A total of 112 elderly patients with dentition loss admitted from October 2018 to December 2022 were selected and divided into research group(n=57)and control group(n=55)by random number table method.The control group received conventional dental implant restoration,and the research group received immediate im-plant placement and restoration with All-on-4.The curative effect,plaque index,bleeding index,probing depth,and the lev-els of inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]and miR-155 in gingival crevicular fluid were compared at 6 months after operation.The masticatory function,retention function,language function,and oral health impact profile-14(OHIP-14)scores were compared between the two groups before and at 6 months after operation.The post-operative complications and 2-year survival rate of restoration with the prosthesis were recorded.Results At 6 months after operation,the total effective rate of the research group was91.23%(52/57),which was higher than 74.55%(41/55)of the control group(P＜0.05),and the total incidence of postoperative complications was 8.77%(5/57),which was lower than 23.64%(13/55)of the control group(P＜0.05).The scores of masticatory function,retention function,and language function were higher than those before operation,and all dimension scores of the OHIP-14 was decreased in both groups at 6 months after operation.The masticatory function,retention function and language function scores of the research group were higher than those of the control group,while all dimension scores of the OHIP-14 was lower than that of the control group(P＜0.05).The plaque index,bleeding index,probing depth,as well as the levels of TNF-α,IL-6,and miR-155 in the gingival crevic fluid of the research group were lower than those of the control group at 6 months after operation(P＜0.05).After 2 years of follow-up,the survival rate of restoration with the prosthesis in the research group was higher than that in the control group(P＜0.05).Conclusion Immediate implant placement and restoration with All-on-4 has significant advantages in the treatment of elderly patients with dentition loss.It can effectively improve the masticatory function,retention function,language function,reduce periodontal inflammation and complications,and improve oral health-related quality of life and the survival rate of restoration with the prosthesis.关键词
