Abstract
Objective:To explore the clinical significance of low-dose CT angiography(CTA)combined with CT perfusion imaging(CTP)in the diagnosis of aneurysmal subarachnoid hemorrhage(aSAH).Methods:A total of 97 patients with aSAH admitted to Zoucheng People's Hospital from January 2021 to January 2024 were selected as the research subjects.Based on whether delayed cerebral hemorrhage(DCI)occurred within 14 days after the onset,the patients were divided into the DCI group and the non-DCI group.All patients underwent low-dose CTA and CTP examinations.The value of low-dose CTA combined with CTP in the diagnosis of aSAH was analyzed.Results:There were 23 cases of DCI within 14 days after onset,with an incidence rate of 23.71%.The results of low-dose CTA showed that the diameter of aneurysms in the DCI group was larger than that in the non-DCI group,and the proportion of irregular aneurysm morphology and cerebral vasospasm was higher in the DCI group than in the non-DCI group,with statistically significant differences(P<0.05).The CTP examination results showed that compared with the non-DCI group,the cerebral blood flow(CBF)and cerebral blood volume(CBV)in the DCI group were smaller,and mean transit time(MTT)and time to peak(TTP)were relatively long,with statistically significant differences(P<0.05).The results of the receiver operating characteristic curve showed that the area under the curve of aneurysm diameter,aneurysm morphology,cerebral vasospasm,CBF,CBV,MTT,TTP,and combined detection in the diagnosis of aSAH were 0.745,0.677,0.704,0.817,0.810,0.769,0.828,and 0.949,respectively,all having certain diagnostic value.Conclusion:Low-dose CTA combined with CTP has important clinical application value in the diagnosis of aSAH patients.It can accurately assess the anatomical characteristics of aneurysms and the degree of cerebral vasospasm,and quantitatively analyze the changes in cerebral hemodynamics,providing a reliable basis for the early identification and intervention of DCI.关键词
