经阴道超声在早期输卵管妊娠中的诊断价值分析

Abstract
Objective:To analyze the application value of transvaginal ultrasound in the diagnosis of early tubal pregnancy and the ultrasound blood flow parameters.Methods:A retrospective analysis was conducted on the data of 120 patients with suspected early tubal pregnancy admitted to Junan County People's Hospital from May 2022 to May 2024.All patients underwent transvaginal ultrasound examination.Taking the final clinical diagnosis result as the gold standard,the patients were divided into the tubal pregnancy group(n=107)and the normal intrauterine pregnancy group(n=13)based on the diagnosis results.Compare the ultrasonic blood flow parameters of the two groups.The receiver operating characteristic(ROC)curve was drawn to analyze the value of ultrasound blood flow parameters in the diagnosis of early tubal pregnancy.Results:The sensitivity of transvaginal ultrasound in diagnosing early tubal pregnancy was 92.50%(99/107),the specificity was 84.62%(11/13),the accuracy was 91.67%(110/120),the positive predictive value was 98.02%(99/101),and the negative predictive value was 57.89%(11/19).The resistance index(RI)and pulsatility index(PI)of the uterine artery in the tubal pregnancy group were both higher than those in the normal intrauterine pregnancy group,the differences were statistically significant(P<0.05).The results of the ROC curve showed that the areas under the curve of the ultrasound blood flow parameters of uterine artery RI,PI and the combination of both for the diagnosis of early tubal pregnancy were 0.890,0.874 and 0.947 respectively,all of which had high diagnostic value,and the combined diagnosis was the highest,with a sensitivity of 0.813 and a specificity of 0.999 Conclusion:Transvaginal ultrasound has a high application value in the diagnosis of early tubal pregnancy.The increase of RI and PI in the uterine artery can be used as an important basis for differential diagnosis,providing reliable objective indicators for the accurate identification of tubal pregnancy in the early clinical stage.关键词
