超声应变弹性成像半定量分析在颅脑肿瘤手术中的应用
Application of Semi-quantitative Analysis of Ultrasound Strain Elastogra-phy in Cranial Tumor Surgery

Abstract
Objective:To perform strain elastography on common intracranial tumors,evaluate tumor boundaries,conduct semi-quantitative analysis of the imaging data,and summarize the characteristics of stiffness differentiation based on differences in mean tissue elasticity(MTE)among various intracranial tumors,so as to provide information for intraoperative resection of tumors.Methods:Fifty patients who underwent brain tumor surgery at the Department of Neurosurgery,Sichuan Cancer Hospital between July 2022 and July 2023 were pro-spectively enrolled in this study.Prior to surgical resection,strain elastography was performed on the tumors and compared with grayscale ultrasound to assess the clarity of tumor boundaries.Eligible images were then subjected to semi-quantitative analysis to obtain MTE values for different intracranial tumors.Differences in MTE values among intracranial tumor tissues were subsequently compared.Results:Strain elastography enhanced tumor boundary delineation in 11 of the 50 tumors.Strain elastography demonstrated superior performance in delineating high-grade glioma(HGG)boundaries compared to grayscale ultrasound,achieving enhanced margin identification in 7 cases(P=0.028).The median MTE values of common intracranial tumors were 75.0(HGG),88.5[low-grade glioma(LGG)],116.5(meningioma),and 104.0(metastatic tumor).Significant differences in MTE were observed between HGG and metastatic tumors(P=0.002),HGG and meningi-omas(P<0.001),and LGG and meningiomas(P=0.019).However,no significant differences were found between LGG and HGG,LGG and metastatic tumors,or metastatic tumors and meningiomas.Conclusion:Strain elastography demon-strates superior tumor margin delineation compared to grayscale ultrasound.MTE values vary significantly among histological subtypes of intracranial tumors.Intraoperatively,surgeons can leverage strain elastography-measured MTE values combined with ultrasound features to assess tumor stiffness and refine resection strategies.关键词
