针药并用治疗卒中后睡眠障碍的疗效观察及对睡眠质量和神经功能缺损的影响
Observation on the efficacy of acupuncture and medication in the treatment of post-stroke sleep disorder and its effect on sleep quality and neurological deficits

Abstract
Objective To observe the clinical efficacy of spirit-regulating and mind-calming acupuncture combined with oral Zolpidem tartrate tablets in the treatment of post-stroke sleep disorders(PSSD)and its effects on sleep quality and neurological deficits.Method Seventy-eight patients with PSSD were divided into an observation group and a control group by random number table method,with 39 cases in each group.The control group was treated with Zolpidem tartrate tablets,and the observation group was treated with the spirit-regulating and mind-calming acupuncture on the basis of the control group.The clinical efficacy of the two groups was compared,and the scores of Athens insomnia scale(AIS),Pittsburgh sleep quality index(PSQI)and National Institutes of Health stroke scale(NIHSS),polysomnography indexes(including number of awakenings,sleep latency,and total sleep time),cerebral hemodynamic parameters[cerebral arterial resistance index(RI),systolic peak flow velocity(Vs),mean peak flow velocity(Vm)and cerebral blood pressure(BP)],and serum neurotransmitters[dopamine(DA),5-hydroxytryptamine(5-HT)]were observed before and after treatment.Result The total effective rate was higher in the observation group(94.9%)than in the control group(71.8%)(P＜0.05).After treatment,the AIS,PSQI,and NIHSS scores of the two groups decreased compared to those before treatment(P＜0.05),and those of the observation group were lower than those of the control group(P＜0.05).After treatment,the number of awakenings and sleep latency of the two groups decreased compared to those before treatment(P＜0.05),and those of the observation group were lower than those of the control group(P＜0.05);the total sleep time of the two groups increased compared to that before treatment(P＜0.05),and that of the observation group was higher than that of the control group(P＜0.05).After treatment,RI of the two groups decreased compared to that before treatment(P＜0.05),and that of the observation group was lower than that of the control group(P＜0.05);the Vs and Vm of the two groups increased compared to those before treatment(P＜0.05),and those of the observation group were higher than those of the control group(P＜0.05).After treatment,the serum DA level of the two groups decreased compared to that before treatment(P＜0.05),and that of the observation group was lower than that of the control group(P＜0.05);the serum 5-HT level of the two groups increased compared to that before treatment(P＜0.05),and that of the observation group was higher than that of the control group(P＜0.05).Conclusion The combination of spirit-regulating and mind-calming acupuncture and Zolpidem tartrate tablets can effectively improve the insomnia symptoms and cerebral hemodynamics of PSSD patients,reduce the degree of neurological impairment,regulate the release of neurotransmitters,and the therapeutic effect is remarkable.关键词
