燔针劫刺法联合中药外敷治疗卒中后Ⅰ期肩手综合征的疗效观察及对静脉血流参数的影响
Therapeutic efficacy of fire needling combined with traditional Chinese medicines external application in the treatment of shoulder-hand syndrome in stage Ⅰ after stroke and its effect on venous blood flow parameters

Abstract
Objective To observe the clinical efficacy of fire needling method combined with the addition and subtraction of Shen Tong Zhu Yu formula external application on the shoulder-hand syndrome in post-stroke stage I and the effect on venous blood flow parameters.Method A total of 110 patients with post-stroke stage I shoulder-hand syndrome were randomly divided into a observation group and a control group,with 55 cases in each group.Both groups were given basic treatment and active and passive exercise,and the control group was given the addition and subtraction of Shen Tong Zhu Yu formula external application,while the observation group was given fire needling treatment on the basis of the control group.The shoulder-hand syndrome score(SHSS),visual analog scale(VAS),swelling,simplified Fugl-Meyer assessment(FMA),Barthel index(BI)scores,internal diameters of the median elbow vein,radial vein and axillary vein,and peak systolic velocity(PSV)changes were observed before and after treatment in the two groups,and the clinical efficacy of the two groups were compared.Result The total effective rate was 96.4%in the observation group and 83.6%in the control group,which was higher in the observation group than in the control group(P＜0.05).The SHSS scores of sensation,autonomic nerve,shoulder abduction,shoulder external rotation and total SHSS scores decreased in both groups after treatment,and those of the observation group were lower than those of the control group,and the difference was statistically significant(P＜0.05).The VAS and swelling scores decreased in both groups after treatment,and those of the observation group were lower than those of the control group,the difference was statistically significant(P＜0.05).The FMA and BI scores increased in both groups after treatment,and those of the observation group were higher than those of the control group,the difference was statistically significant(P＜0.05).The PSV of the median elbow vein,radial vein and axillary vein increased in both groups after treatment,and that of the observation group was higher than that of the control group,and the difference was statistically significant(P＜0.05);the internal diameter of the median elbow vein,radial vein and axillary vein decreased in both groups,and that of the observation group was lower than that of the control group,and the difference was statistically significant(P＜0.05).Conclusion On the basis of basic treatment and active and passive exercise,fire needling combined with the addition and subtraction of Shen Tong Zhu Yu formula external application is effective in the treatment of post-stroke stage I shoulder-hand syndrome,which can effectively reduce the clinical symptoms and swelling and pain of the upper limbs,improve the upper limb movement disorders,enhance the venous blood circulation,and improve the ability of self-care.关键词
