经阴道超声血流参数联合血清IL-12水平对PCOS不孕症IVF-ET妊娠失败的预测价值
Predictive value of transvaginal ultrasound blood flow parameters combined with serum interleukin 12 level for pregnancy failure of IVF-ET among infertility women with PCOS

Abstract
Objective:To explore the synergic efficacy of transvaginal ultrasound blood flow parameters and serum interleukin-12(IL-12)in predicting pregnancy failure of IVF-ET among infertile women with PCOS. Methods:A total of 148 PCOS infertile patients treated with IVF-ET from July 2022 to July 2024 in Wuhan Traditional Chinese Medicine Hospital were selected as the study objects,and then divided into pregnancy success group(pregnancy success,n=60)and pregnancy failure group(pregnancy failure,n=88)according to pregnancy outcomes.Uterine hemodynamic parameters including resistance index(RI),pulsation index(PI),ratio of end-systolic to end-diastolic maximum blood flow velocity(S/D)were detected by vaginal color Doppler ultrasonography in all patients,and serum IL-12 level was detected by enzyme-linked immunosorbent assay(ELISA).The levels of IL-12 and uterine hemodynamic parameters between the two groups were compared.Multivariate logistic regression analysis was conducted to analyze the influencing factors of pregnancy failure in IVF-ET among infertile women with PCOS,and the receiver operating characteristics(ROC)curve was used to analyze the predictive value of transvaginal ultrasound blood flow parameters combined with serum IL-12 level in IVF-ET pregnancy failure among PCOS infertile women. Results:When compared with the pregnancy success group,the testosterone(T)level[(6.98±0.78)nmol/L vs.(5.65±0.63)nmol/L],vaginal ultrasound blood flow parameters(RI,PI,S/D),serum IL-12 level[(17.65±2.14)ng/ml vs.(15.43±1.88)ng/ml]were significantly increased in the pregnancy failure group(P＜0.05).Multivariate logistic regression analysis showed that T[OR=2.685,95％CI(2.061,3.498)],IL-12[OR=3.748,95％CI(2.174,6.463)],RI[OR=5.054,95％CI(2.995,8.529)],PI[OR=4.108,95％CI(2.572,6.563)],S/D[OR=3.886,95％CI(2.590,5.830)]levels were the risk factors of pregnancy failure in infertile PCOS women undergoing IVF-ET(P＜0.05).The test performance evaluation showed that the area under the curve(AUC)of transvaginal ultrasound blood flow parameters(RI,PI,S/D)combined with serum IL-12 level in predicting pregnancy failure in IVF-ET was higher than that of RI(Z=4.485),PI(Z=5.049),S/D(Z=4.351)and IL-12 level(Z=4.441),showing significant differences(P＜0.05). Conclusions:The serum level of IL-12 is high in PCOS infertile women with pregnancy failure in IVF-ET,and the value of transvaginal ultrasound blood flow parameters combined with serum IL-12 level in predicting pregnancy failure among PCOS infertile women undergoing IVF-ET is high.关键词
