Analysis of the pathogenesis of"yang deficiency and blood stasis"in endometriosis and clinical practice of staged treatment with warming yang and activating blood

Abstract
Endometriosis(EMT)is a common and frequently-occurring disorder among women of reproductive age.The main clinical manifestations are dysmenorrhea and infertility,which seriously affect the quality of life of patients.Traditional Chinese Medicine(TCM)has unique advantages in regulating holistic bodily states,alleviating clinical symptoms,and reducing the recurrence rate of EMT.Based on the TCM of"Zhongyang"thought and"yang transforming qi,yin shaping up body"theory,this paper proposes that the core pathogenesis of EMT is"yang deficiency and blood stasis",and the deficiency of yang qi leads to dysfunctional qi transformation,the gradual accumulation of yin transformed into ectopic lesions.Professor Lou Yiyun of Hangzhou Hospital of Traditional Chinese Medicine established the staged treatment based on disease differentiation system of"warming yang and activating blood"based on her gynecological diagnosis and treatment concept of"yangmi guyin".According to the menstrual cycle,the drug regimen is dynamically adjusted,that is,nourishing yin and blood to consolidate foundation during the post menstrual period,warming kidney and activate blood to decorporation during the intermenstruum,soothing liver qi,warm unblock and regulating Chong meridian in the premenstrual period,promoting blood circulation and removing blood stasis by aligning with the physiological trend to eliminate zheng during menstruation.A representative case study of EMT patients with yang deficiency of spleen and kidney and blood stasis demonstrated that the combination of warming yang herbs(such as Astragalus membranaceus,Radix Morindae Officinalis)with activating blood agents,supplemented by carbonized herbs for consolidating thoroughfare channel,hemostasis and Sanjie Capsules to resolve stasis and promote regeneration,effectively normalized menstrual flow and alleviated dysmenorrhea after four treatment cycles.These outcomes validate the dual therapeutic effects of reinforcing healthy qi to eliminate pathogenic factor.This paper systematically elucidates the TCM pathological mechanism of"yang deficiency and blood stasis"in EMT,and innovatively establishes a diagnostic-treatment model integrating cyclical therapy with classical theories,so as to provide both theoretical foundations and practical references for TCM management of refractory gynecological disorders.关键词
