Abstract
Objective To investigate the increased infarct volume and its influencing factors in patients with acute ischemic stroke(AIS)after endovascular therapy(EVT).Methods A total of 254 patients with AIS admitted to Haimen District People's Hospital from March 2021 to February 2023 were taken as the research object.All patients received EVT after admission,and were divided into the enlarged group and the non-enlarged group based on the presence or absence of increased infarct volume 24 hours after surgery.General data and related examination results of patients in the two groups were analyzed.Multivariate Logistic regression was used to analyze the related factors affecting the increase in infarct volume,and the receiver operating characteristic(ROC)curve was drawn to analyze the predictive efficacy of the influencing factors on the increase in infarct volume.Results After treatment,the infarct area and the increase in infarct area in the enlarged group were significantly greater than those in the non-enlarged group(both P<0.05).The age and baseline infarct area in the enlarged group were larger than those in the non-enlarged group,and the proportions of diabetes and postoperative mTICI grade<2b in the enlarged group were higher than those in the non-enlarged group,Fazekas score,National Institutes of Health Stroke Scale score at admission,and hypoperfusion intensity ratio(HIR)score in the enlarged group were higher than those in the non-enlarged group,with statistically significant differences(P<0.05).Logistic regression analysis showed that age,baseline infarct area,Fazekas score,HIR score and postoperative mTICI grade were the influencing factors for the increase in infarct volume after EVT(all P<0.05).ROC curve based on influencing factors showed that the area under the curve(AUC)for HIR score was 0.923;for Fazekas score,it was 0.795;for age,it was 0.808;for postoperative mTICI grade,it was 0.706;and for baseline infarct area,it was 0.900(all P<0.05).Conclusion Age,baseline infarct area,Fazekas score,HIR score,and postoperative mTICI grade are all influencing factors for the increase in infarct volume after EVT for cerebral infarction,and the above factors have good predictive efficacy for the increase in infarct volume.关键词
