Drug Distribution and Safety of Dexamethasone Injection after Extracting Perilympha through Round Window

Abstract
Objective Study of the maximum safe volume for lymphatic fluid extraction from the inner ear of guinea pigs,safety and efficacy of the procedure of extracting perilympha and then injecting drugs through a round window membrane(RWM).Methods Fifteen normal guinea pigs were taken and randomized into groups,and the maximum safe volume of outer lymphatic fluid to be extracted was determined by gradient extraction of outer lymphatic fluid from the inner ear followed by audiometry.Afterwards,20 normal guinea pigs were randomly divided into 3 groups:a blank control group,a systemic drug administration group,and an extracting perilympha and then injecting drugs via RWM group(group injecting the same volume of dexamethasone after via RWM extracting perilympha with the maximum safe extraction volume).(1)Efficacy experiment:the drug distribution efficiency was determined by comparing the fluorescence staining expression of the drug in the inner ear 2h after treatment.(2)Safety experiment:the safety of the operation was determined by comparing the mean thresholds of each frequency band of ABR 24 h after treatment.Results The maximum safe dose for guinea pig lymphatic fluid extraction through the inner ear was 4 μL.Dexamethasone was more efficiently distributed in the group of extracting perilympha and then injecting drugs via RWM.The mean ABR thresholds of the 24h postoperative in the extracting perilympha and then injecting drugs via RWM group had a short-term rise only at 32 kHz and did not affect the hearing thresholds at other frequencies.Conclusion The method of extracting perilympha and then injecting drugs via RWM can realize the efficient distribution of inner ear drugs with short-term damage to high-frequency hearing only.关键词
