Abstract

Objective To investigate the impact of intravascular injection of two cosmetic fillers,namely hyaluronic acid and Ellanse,on the severity of embolism.Methods The rabbit auricular artery was selected as the experimental model to establish a flap necrosis model by ligating three dorsal auricular flaps,followed by intravascular embolization induced by injecting equal amounts of hyaluronic acid and Ellansé into the artery.Fluorescence labeling and in vivo imaging were employed to monitor filler displacement during injection,while flap necrosis and blood flow at the ischemic site were assessed immediately after surgery,on day 3 and day 7 post-surgery.H&E histological analysis was conducted to evaluate filler degradation and local inflammatory response.Results In vivo imaging of small animals revealed that compared to Ellansé,hyaluronic acid fillers were more widely dispersed in the ear margin after intravascular injection and more prone to displacement within blood vessels.Laser speckle imaging demonstrated a rapid decrease in flap blood flow to 7.65%±0.37%of initial levels following ligation,with recovery to 64.75%±7.29%in the control group at 7 days post-operation;however,blood flow recovery was significantly impeded in both the hyaluronic acid and Ellansé groups(47.83%±6.41%and 46.56%±2.11%,respectively).Gross observation indicated local flap necrosis at the distal end after 7 days of intravascular injection with either hyaluronic acid or Ellansé,while histological analysis of H&E staining showed vessel expansion following injection with incomplete tamponade degradation observed at the distal end of the auricular artery for both substances,indicating their propensity for shifting within blood vessels.Conclusions Compared to Ellansé,hyaluronic acid exhibits a higher susceptibility to intravascular translocation following intravascular injection.Both substances can induce severe vascular obstruction and reduce blood flow velocity.Macroscopic examination revealed that this type of intravascular blockage leads to local flap necrosis.Histological evaluation using H&E staining demonstrated that both hyaluronic acid and Ellansé cannot be completely degraded within the blood vessel without external intervention,thereby causing certain damage to the vasculature.
