Abstract
Objective To explore the influence of calcium ion concentration in dialysate on microinflammatory status and heart disease in end-stage renal disease(ESRD)patients underwent maintenance hemodialysis(MHD).Methods A total of 120 ESRD patients who received MHD treatment in Nantong Haimen District People's Hospital from September 2022 to September 2023 were divided into dCa2+1.25 group,dCa2+1.50 group,dCa2+1.75 group respectively according to random number table.After 12 MHD three groups oxidative stress indexes[oxygen free radicals(ROS),advanced oxidation protein products(AOPP)],inflammation indexes[tumor necrosis factor alpha(TNF-alpha),interleukin 1(IL-1),interleukin 6(IL-6)],myocardial injury markers[myoglobin(Mb),cardiac troponin I(cTnI),creatine kinase isoenzyme(CK-Mb),lactate dehydrogenase(LD)],cardiac function indicators[stroke output(SV),left ventricular ejection fraction(LVEF),left ventricular short-axis shortening rate(FS)]were compared,and abnormal occurrence of calcium and phosphorus metabolism during the recording period were recorded.Results After 12 times of MHD,serum levels of ROS,AOPP,TNF-α,IL-1,IL-6,Mb,cTn,CK-MB,LD in three groups were dCa2+1.25 group,dCa2+1.50 group and dCa2+1.75 group from low to high(P<0.05).There were no significant difference in SV,LVEF and FS between three groups(P>0.05).During the period of MHD,incidence of abnormal blood calcium and blood phosphorus in the dCa2+1.50 group were lower than that in dCa2+1.25 group and the dCa2+1.75 group(P<0.05).Conclusion For patients with normal serum calcium and phosphorus metabolism before dialysis treatment,low calcium ion level in dialysate is helpful to alleviate the microinflammatory state and myocardial injury caused by continuous dialysis,but there is a risk of abnormal calcium and phosphorus metabolism after long-term use.关键词
