两种修补术在嵌顿性腹股沟疝手术治疗中的效果对比
Comparison of Effect of Two Hernioplasty in the Surgical Treatment of Incarcerated Inguinal Hernia

Abstract
Objective:To compare the efficacy of two different hernioplasty in the surgical treatment of incarcerated inguinal hernia.Method:The clinical data of 78 patients who received emergency surgery for adult incarcerated inguinal hernia in Ji'an Central People's Hospital were retrospectively analyzed from January 2013 to January 2018.According to different hernioplasty,the patients were divided into traditional hernia repair(Bassini)group(33 cases)and tension-free hernia repair(Lichtenstein)group(45 cases).The surgical time,intraoperative blood loss,postoperative exhaust time and hospital stay were compared between groups,and the inflammatory indicators[white blood cell(WBC),neutrophil(NEUT),C reactive protein(CRP)],serum pain mediators[β-endorphin(β-EP),substance P(SP),nitric oxide(NO)],complications and occurrence were observed in both groups.Result:There were no statistical differences in surgical time,intraoperative blood loss,postoperative exhaust time and hospital stay between groups(P>0.05).At 1 d after surgery,there were no significant differences in WBC,NEUT and CRP between the two groups(P>0.05).At 3 d after surgery,the levels of β-EP,SP and NO in Lichtenstein group were significantly lower than those in Bassini group,with statistical significance(P<0.05).After 5 years of postoperative follow-up,there was no statistical significance in the incidence of postoperative complications between the two groups(P>0.05),and the recurrence rate at 5 years after surgery of Lichtenstein group was lower than that of Bassini group,with statistical significance(P<0.05).Conclusion:In the treatment of incarcerated inguinal hernia with Lichtenstein surgery,the serum pain mediators indexer of patients are lower than those of traditional hernia repair,and the 5 years after surgery recurrence rate is lower.关键词
