Clinical Application of Dynamic Monitoring of Inferior Vena Cava Collapse Index in Fluid Treatment of Severe Pneumonia Complicated with Heart Failure in Infants

Abstract
Objective:To explore the clinical application of dynamic monitoring of inferior vena cava collapse index in the treatment of severe pneumonia complicated with heart failure in infants.Method:A total of 80 infants with severe pneumonia complicated with heart failure admitted to Jiujiang Maternal and Child Health Hospital from January to May 2024 were selected and divided into control group(conventional treatment alone)and study group(fluid treatment guided by the dynamic monitoring of inferior vena cava collapse index on the basis of the control group)according to random number table method,with 40 cases in each group.The clinical efficacy,clinical outcome,cardiac function index and arterial blood gas index of the two groups were compared.Result:The total clinical effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).The hospitalization time of the study group was shorter than that of the control group,and the hospitalization cost was less than that of the control group,the differences were statistically significant(P<0.05).After treatment,left ventricular ejection fraction(LVEF),stroke volume(SV),cardiac output(CO),cardiac index(CI),pulse oximetry satiety(SpO2)and arterial partial oxygen pressure(PaO2)in both groups were higher than those before treatment,while heart rate(HR)and partial pressure of carbon dioxide(PaCO2)were lower than those before treatment,LVEF,SV,CO,CI,SpO2 and PaO2 in the study group were higher than those in the control group,while HR and PaCO2 were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The effect of dynamic monitoring of inferior vena cava collapse index to guide the treatment of infants with severe pneumonia complicated with heart failure fluid is significant,and the clinical outcome,heart function and arterial blood gas level of infants are significantly improved.关键词
