Abstract
Objective:To explore the clinical effect of AcoStream peripheral blood thrombus aspiration catheter thrombectomy combined with Reteplase thrombolytic therapy in patients with high-risk pulmonary embolism.Method:A total of 90 patients with high-risk pulmonary embolism admitted to Yantai Affiliated Hospital of Binzhou Medical University from June 2023 to August 2024 were selected as the research subjects and divided into the control group and the experimental group by the random number table method,with 45 cases in each group.The control group received Reteplase thrombolytic therapy,while the experimental group was treated with AcoStream peripheral blood thrombus aspiration catheter thrombectomy on the basis of the control group.The changes of pulmonary artery pressure[pulmonary artery systolic pressure(PASP),pulmonary artery diastolic pressure(PADP)],coagulation function indicators[prothrombin time(PT),D-dimer(D-D),fibrinogen(FIB)],and myocardial injury indicators[creatine kinase isoenzyme(CK-MB),cardiac troponin I(cTnI)]in the two groups were observed.The therapeutic effects of the two groups were evaluated.Result:One week after the treatment,the levels of PASP and PADP in the experimental group were both lower than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the experimental group was higher than that in the control group,the difference was statistically significant(P<0.05).One week after the treatment,PT in the experimental group was longer than that in the control group,and both D-D and FIB were lower than those in the control group,the differences were statistically significant(P<0.05).One week after the treatment,the levels of CK-MB and cTnI in the experimental group were both lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:On the basis of Reteplase thrombolytic therapy,AcoStream peripheral blood thrombus aspiration catheter thrombectomy for high-risk pulmonary embolism patients can effectively remove thrombus,reduce pulmonary artery pressure,improve coagulation function,enhance overall efficacy,and the combination therapy is beneficial for reducing myocardial injury.关键词
