Abstract
Objective:To investigate the efficacy of continuous veno-venous hemodiafiltration combined with hemoperfusion(HP)in the treatment of patients with severe acute organophosphate pesticide poisoning(AOPP)and its impact on myocardial injury.Method:A total of 85 patients with severe AOPP treated in Jiangsu Province(Suqian)Hospital from January 2022 to January 2024 were selected as study objects and divided into group B(n=42)and group A(n=43)according to random number table method.Group B was treated with HP,and group A was treated with continuous veno-venous hemodiafiltration combined with HP.The clinical efficacy,liver and renal function,inflammatory factor levels,myocardial injury,complications of the two groups were observed.Result:After 5 d of treatment,the total effective rate of group A(93.02%)was higher than that(76.19%)of group B,the difference was statistically significant(P<0.05).Levels of liver and renal function indexes[total bilirubin(TBIL),direct bilirubin(DBIL),blood urea nitrogen(BUN),serum creatinine(Scr)]of both groups were decreased,and those in group A were lower than those in group B,the differences were statistically significant(P<0.05).Levels of inflammatory factor indexes[white blood cells(WBC),interleukin(IL)-6,C reactive protein(CRP),tumor necrosis factor(TNF)-α]in both groups were decreased,and those in group A were lower than those in group B,the differences were statistically significant(P<0.05).Myocardial injury[serum troponin T(cTnT),creatine kinase isoenzyme(CK-MB)and B-type natriuretic peptide precursor(Pro-BNP)]were significantly improved in both groups,and those in group A were significantly lower than those in group B,the differences were statistically significant(P<0.05).The incidence of complications in group A(6.98%)was lower than that(26.19%)in group B,the difference was statistically significant(P<0.05).Conclusion:Continuous vene-venous hemodiafiltration combined with HP treatment can improve liver and kidney function,reduce inflammatory cytokine levels,alleviate myocardial injury,improve clinical efficacy,and reduce the incidence of complications in patients with severe AOPP.关键词
