经桡动脉入路在颈动脉狭窄支架手术中的应用
Application of Transradial Acess in Carotid Artery Stenosis Stent Surgery

Abstract
Objective:To explore the application of the transradial access(TRA)in carotid artery stenosis stent surgery.Method:A retrospective analysis was conducted on clinical data of 102 patients who underwent carotid artery stenosis stent surgery at Linfen People's Hospital from February 2022 to February 2024.According to different approaches,they were divided into two groups.Among them,50 patients who underwent transfemoral access(TFA)surgery were included in the TFA group,and 52 patients who underwent TRA surgery were included in the TRA group.The perioperative indicators,levels of cerebral blood flow perfusion parameters[cerebral blood flow(CBF),cerebral blood volume(CBV),time to peak(TTP),mean transit time(MTT)],incidence of complications at the puncture site and comfort scores were compared between the two groups.Result:The operation time and fluoroscopy time in the TRA group were both shorter than those in the TFA group(P<0.05).There was no statistically significant difference in the one-time puncture success rate between the two groups(P>0.05).One month after the operation,the CBF and CBV in both groups were higher than those before the operation,while the TTP and MTT were lower than those before the operation(P<0.05).There were no statistically significant differences in cerebral blood flow perfusion parameters between the two groups(P>0.05).The incidence rate of complication of the puncture site in the TRA group was 5.77%,which was lower than 20.00%in the TFA group(P<0.05).At 6,12 and 24 h after the operation,the scores of the operation position comfort questionnaire(OPCQ)in the TRA group were all higher than those in the TFA group(P<0.05).Conclusion:TRA can improve the surgical efficiency in carotid artery stenosis stent surgery,promote the recovery of cerebral blood perfusion,reduce the incidence rate of complication at the puncture site,and significantly enhance the patient's comfort level.关键词
