使用混合编织移植物行MPFL重建术治疗复发性髌骨脱位患者的临床效果
Clinical Outcomes of Using Hybrid Grafts for MPFL Reconstruction in Patients with Recurrent Patellar Dislocation

Abstract
Objective:To compare the short-and medium-term outcomes of hybrid grafts versus autologous grafts for medial patellofemoral ligament(MPFL)reconstruction in patients with recurrent dislocation of patellar and to analyze the potential reasons for any differences.Method:This study was a prospective randomized controlled trial design.Patients with recurrent dislocation of patellar who underwent MPFL reconstruction at Beijing Renhe Hospital from January 2020 to January 2023 were included.Finally,a total of 23 patients were enrolled and randomly divided into two groups,including 11 cases in autologous grafts group and 12 cases in hybrid grafts group.The surgical approach involved MPFL reconstruction with or without tibial tubercle osteotomy,the hybrid grafts group received hybrid grafts,while the autologous grafts group received autologous grafts.The subjective knee scores before operation,3 months after operation,6 months after operation and at the last follow-up were compared.The imaging data before operation,immediately after operation and at the last follow-up were compared.Result:There were no significant differences in the subjective knee scores[patellofemoral joint Kujala scores and international knee documentation committee(IKDC)scores]between the two groups before operation(P>0.05).Compared with those before operation,the subjective knee scores of the two groups were significantly improved at 3 months,6 months,and last follow-up(P<0.05).At 3 and 6 months after operation,the subjective scores of knee joint in the hybrid graft group were better than those in the autograft group(P<0.05).However,at the final follow-up,there were no significant differences in scores between the two groups(P>0.05).At immediate after operation and last follow-up,there were no significant differences in imaging parameters[tibial tuberosity trochlear groove distance(TTTG),patellar lateral tilt angle(PLT),patelle trochlear groove distance(PTG)]between the two groups(P>0.05),and compared with those before operation,the postoperative imaging data were significantly improved(P<0.05).Conclusion:Hybrid grafts is a favorable option for MPFL reconstruction in patients with recurrent dislocation of patellar.Compared to autologous grafts,hybrid grafts provides better short-term outcomes and allow for earlier rehabilitation.关键词
