Abstract
Objective To investigate the application value of healthcare failure mode and effect analysis(HFMEA)in medication guidance for patients with chronic kidney disease(CKD).Methods An 8-member HFMEA management team was established,by which the HFMEA on the six main processes of medication guidance for CKD patients,including patient assessment,issuance of medical orders,prescription review,medication administration,exercise and dietary guidance,and follow-up were conducted then,analyzed the failure modes,sorted out and analyzed the causes of failure,formulated and implemented improvement measures.Finally the risk priority number(RPN)values and patient satisfaction before and after the improvement of HFMEA were compared.Results A total of nine failure modes were screened out,including 1,1,5 and 2 items for low risk,medium risk,high risk and very high risk,respectively.After the improvement,the overall RPN value of the 9 failure modes was reduced from 319 to 104,with a decrease of 67.40%.The patient satisfaction score raised from(38.31±6.41)points to(46.17±7.02)points,and the overall satisfaction raised from 60.00%to 90.00%(P<0.05).Conclusion By adopting HFMEA method,it is convenient for the identification and improvement of failure modes in the medication guidance process for CKD patients,which is beneficial for ensuring the safety and effectiveness of medication and improving patients'satisfaction.关键词
