北京某社区4种常见慢性病患者用药依从性与影响因素研究
Study on Medication Compliance and Influencing Factors of Patients with Four Common Chronic Diseases in a Community in Beijing

Abstract
Objective To investigate the status of medication compliance in patients with four chronic diseases(hypertension,diabetes,hyperlipidemia and hyperuricemia)in community,and to analyze the influencing factors.Methods Convenience sampling method was used to select patients with at least one chronic disease diagnosed in a community hospital in Beijing receiving pharmacotherapy for≥3 months.Questionnaires were distributed and collected on the spot,and data were extracted and analyzed.Results A total of 800 valid questionnaires were collected.The order of medication compliance of patients with four chronic diseases was hyperuricemia(68.50%)>hyperlipidemia(53.50%)>diabetes(45.00%)>hypertension(41.50%).For patients with hypertension,those who participated in health education[OR=3.823,95%CI(1.444,10.121),P=0.007]had better compliance;smoking[OR=0.126,95%CI(0.035,0.450),P=0.001],drinking[OR=0.214,95%CI(0.055,0.843),P=0.027],disease duration≤1 year[OR=0.016,95%CI(0.001,0.221),P=0.002],occasional monitoring of blood pressure[OR=0.141,[OR=5.830,95%CI(1.933,17.581),P=0.002]had better compliance.Smoking[OR=0.204,95%CI(0.060,0.699),P=0.011]was an independent risk factor for medication compliance.For patients with hyperuricemia,female patients[OR=0.248,95%CI(0.073,0.838),P=0.025],understanding the gout-related knowledge[OR=5.161,95%CI(1.907,13.967),P=0.001],using only one uric acid-lowering drug[OR=3.967,95%CI(1.091,14.434),P=0.036]had better compliance.Patients with disease duration≤1 year had better compliance than those with disease duration≥10 years[OR=4.793,95%CI(1.062,21.640),P=0.042].Conclusion There is a convergence phenomenon among the medication compliance of patients with four kinds of chronic diseases.The medication compliance of patients with chronic diseases in the community is low,and clinical should carry out the medication intervention according to the main factors affecting patients'compliance.关键词
