Efficacy Evaluation of Anti-Infection Optimization Regimen of Piperacillin Sodium and Tazobactam Sodium in the Treatment of Patients with Augmented Renal Clearance

Abstract
Objective To investigate the effect of piperacillin sodium and tazobactam sodium anti-infection optimization regimen in the treatment of patients with augmented renal clearance(ARC).Methods ARC patients were identified by using Cockcroft-Gault(CG)formula combined with ARC SCORE and ARCTIC SCORE scoring system.From January to December 2022 and January to December 2023,35 cases and 50 cases of patients with severe infection were selected as the control group and the observation group respectively.The patients in the control group was treated with routine treatment regimen of piperacillin sodium and tazobactam sodium for injection(4.5 g piperacillin sodium and tazobactam sodium for intravenous drip,once every 8 hours),while the patients in observation group was treated with the optimized scheme of piperacillin sodium and tazobactam sodium(according to Monte Carlo simulation,the initial administration scheme of piperacillin sodium and tazobactam sodium was optimized to the upper limit of safety of 4.5 g,once every 6 hours,and the infusion time was extended to 4 hours).Both groups of patients were treated for 10-14 days.Results After treatment,the body temperature,infection index,clinical symptom score and clinical pulmonary infection score of the two groups were significantly decreased,and the above indexes in the observation group were significantly lower than those in the control group(P<0.05).The effective rate of anti-infection treatment in the observation group was 92.00%,and the eradication rate of pathogenic bacteria was 68.00%,which were significantly higher than 60.00%and 42.86%in the control group(P<0.05).The proportion of antibiotics upgraded to special grade in the observation group was 46.00%,which was significantly lower than 65.71%in the control group(P<0.05).Conclusion Using CG formula method combined with ARC SCORE and ARCTIC SCORE scoring system to identify ARC patients has a significant impact on the pharmacokinetics/pharmacokinetics(PK/PD)compliance rate of piperacillin sodium and tazobactam sodium cleared by kidney.The optimized treatment plan of piperacillin sodium and tazobactam sodium can improve the therapeutic effect,optimize the clinical symptoms,infection indicators and prognosis of patients.关键词
