达格列净联合甘精胰岛素和门冬胰岛素治疗老年糖尿病肾病对患者氧化应激和内皮功能的影响
Effect of Dapagliflozin Combined with Insulin Glargine and Insulin Aspart on Oxidative Stress and Endothelial Function in the Treatment of Elderly Patients with Diabetic Nephropathy

Abstract
Objective To investigate the clinical efficacy of Dapagliflozin combined with insulin glargine and insulin aspart in the treatment of elderly patients with diabetic nephropathy(DN).Methods A total of 128 elderly patients with diabetic nephropathy admitted to the hospital from February 2022 to April 2024 were selected and divided into the observation group(63 cases)and the control group(65 cases)according to the random number table method.Both groups received subcutaneous injection of Insulin Glargine Injection and Insulin Aspart Injection and the observation group was additionally given Dapagliflozin Tablets.Both groups were treated continuously for 12 months.Results The total effective rate in the observation group was 92.06%,which was significantly higher than 76.92%in the control group(P<0.05).After treatment,The levels of fasting blood glucose(FBG),2 hour postprandial blood glucose(2 hPG),glycated hemoglobin(HbA1c),malondialdehyde(MDA),8-hydroxy-2'-deoxyguanosine(8-OHdG),blood urea nitrogen(BUN),24-hour urinary albumin excretion rate(24 hUAE),vascular endothelial growth factor(VEGF),von Willebrand factor(vWF),and endothelin-1(ET-1)in the observation group were significantly lower than those in the control group(P<0.05),while uperoxide dismutase(SOD),glutathione peroxidase(GSH-Px),and estimated glomerular filtration rate(eGFR)were significantly higher than those in the control group(P<0.05).The incidence of adverse reactions was comparable between the observation group and the control group(P>0.05).Conclusion The treatment of dapagliflozin combined with insulin glargine and insulin aspart with elderly patients with diabetic nephropathy can significantly reduce blood glucose and oxidative stress levels,improve renal function,and promote vascular endothelial cell growth.关键词
