临床药师参与1例固定性药疹治疗实践
Clinical Pharmacists′ Practice in the Treatment of Fixed Drug Eruption:A Case Report
摘要
Abstract
Objective To provide a reference for the clinical diagnosis and treatment of fixed drug eruption.Methods The clinical pharmacist's involvement in the diagnosis and treatment of a fixed drug eruption in a patient with pneumonia was reviewed.After admission,the patient was given Piperacillin Sodium and Tazobactam Sodium for Injection 4.5 g once every 8 hours for anti infection treatment;On the second day of admission,the patient developed scattered light red rashes on the back and buttocks,and was given topical application of Mometasone Furoate Cream;On the third day of admission,the patient's infection indicators improved,but the rash did not show significant improvement.After consulting,the dermatologist recommended to confirm whether it was caused by medication and applied Ethacridine Lactate Solution around the mouth for antibacterial purposes.During the clinical pharmacist's consultation,the patient's medical history was reviewed and a similar rash in the same area was found three years ago,which also occurred after using Piperacillin Sodium and Tazobactam Sodium for Injection,with only slight differences in timing and severity,and the clinical pharmacist recommended to discontinue this drug.Results The clinical physician adopted the advices of the dermatologist and the clinical pharmacist,stopped using Piperacillin Sodium and Tazobactam Sodium for Injection,and combined with anti allergic drugs,anti-inflammatory drugs,glucocorticoids,and other treatments.The patient's rash gradually improved before being discharged.The final diagnosis was fixed drug rash,and the patient and their family were reminded to discontinue the use of Piperacillin Sodium and Tazobactam Sodium for Injection for life.Conclusion The participation of the clinical pharmacist had accelerated the exposure process of finding problematic drugs,and it is reminded that clinical physicians should pay close attention to the patient's medication history when encountering special drug rashes.关键词
