高危型人乳头瘤病毒和宫颈病变与阴道微生态的相关性分析
Correlation analysis between high-risk human papillomavirus,cervical lesions and vaginal microbiota

Abstract
Objective To investigate the correlation between high-risk human papillomavirus(HR-HPV),cervical lesions,and changes in various indices of vaginal microbiota,and to identify common subtypes of HR-HPV infection in two groups of subjects.Methods Female who underwent thinprep cytologic test(TCT)and human papillomavirus(HPV)test at the Department of Gynecology Outpatient and Physical Examination Center of the No.2 People's Hospital of Lanzhou from April 2021 to April 2023 were selected as the research subjects.TCT+HPV was used for cervical cancer screening.For those with HR-HPV and TCT abnormalities,further vaginal colposcopy was performed,and biopsy was taken from suspected lesions.Patients with cervical squamous intraepithelial lesions(SIL)were selected as the study group(n=60),and HPV negative and cervical lesions free subjects during the same period were selected as the control group(n=60).All subjects underwent vaginal microbiota test,and various indices of vaginal microbiota were compared between the two groups.Results For patients with HR-HPV,the positivity rate for H2O2(hydrogen peroxide)was 96.67%,the positivity rate for proline aminopeptidase was 75.00%,the positivity rate for N-acetyl-beta-D-glucosaminidase was 85.00%,and the abnormality rate for lactobacillus was 81.67%.Additionally,there was an abnormality rate of 30.00%for cleanliness,and a positivity rate of 31.67%for chlamydia,with statistically significant differences(P<0.05).For patients with HR-HPV,the positivity rate for leukocyte esterase was 65.00%,the positivity rate for sialidase(neuraminidase)was 41.67%,the positivity rate for trichomonas infection was 21.67%,and the positivity rate for candida infection was 28.33%,without statistically significant differences(P>0.05).In the study group,the detection rates for each subtype were as follows,which were 8.33%for type 16,6.67%for type 18,1.67%for type 31,13.33%for type 33,1.67%for type 52,20.00%for type 53,25.00%for type 58,1.67%for type 66,and 13.33%for type 68.In the control group,the detection rates for each subtype were as follows,which were 1.67%for type 16,3.33%for type 18,1.67%for type 31,1.67%for type 33,1.67%for type 52,5.00%for type 53,8.33%for type 58,1.67%for type 66,and 10.00%for type 68.Conclusion HR-HPV infection is closely related to H2O2,proline aminopeptidase,acetyl glucosidase,lactobacilli,cleanliness,chlamydia,and other factors in the vaginal microbiota.There is no significant correlation with leukocyte esterase,sialidase,trichomonas infection,etc.indicating that HR-HPV is an important factor in causing cervical lesions and vaginal microbiota disorders.The subtypes with higher detection rates in the study group were 58,53,68,and 33,while the subtype with higher detection rates in the control group was 68,indicating that the detection rates of HR-HPV subtypes 58,53,68,and 33 were higher.关键词
