Impacts of acupoint electrical stimulation and transverse abdominal muscle block on bladder discomfort and inflammatory factors after prostatectomy

Abstract
Objective To investigate the clinical efficacy and impacts on inflammatory factors of transcutaneous acupoint electrical stimulation combined with transverse abdominal plane block in relieving urinary catheter-related bladder discomfort after transurethral holmium laser enucleation of prostate(HoLEP)in elderly patients with prostatic hyperplasia.Methods A total of 160 patients who underwent transurethral HoLEP under general anesthesia in the Affiliated Hospital of Shandong Second Medical University from March 2023 to April 2024 were selected,and they were divided into the control group,the acupoint electrical stimulation group,the transverse abdominal block group and the acupoint electrical stimulation combined with transverse abdominal block group according to the random number table method,with 40 cases each group.The incidence and severity of urinary catheter-related bladder discomfort in 4 groups were observed at T1(after surgery),T2(leave anesthesia recovery room),T3(24 hours after surgery)and T4(48 hours after surgery).The levels of inflammatory factors before and T3 were also observed.Results The total incidences of urinary catheter-related bladder discomfort at T2 and T3 in the acupoint electrical stimulation combined with transverse abdominal block group were lower than those in the control group,the acupoint electrical stimulation group and the transverse abdominal block group,with statistically significant differences(P<0.05).The levels of interleukin-6,tumor necrosis factor-α and C-reactive protein in the acupoint electrical stimulation combined with transverse abdominal block group in T3 were all lower than those in the control group,the acupoint electrical stimulation group and the transverse abdominal block group,with statistically significant differences(P<0.05).Conclusion Transcutaneous acupoint electrical stimulation combined with transverse abdominal plane block can significantly reduce the incidence of urinary catheter-related bladder discomfort and the incidence of moderate and severe bladder discomfort within 24 hours after prostatectomy,and reduce the level of inflammatory factors and surgical stress.关键词
