Abstract

Objective:Taking the whole-liver proton density fat fraction(PDFF)measured by MRI as the standard,to analyze the consistency between PDFF and the liver fat fraction(FF)in water-phase and fat-phase maps measured by MRI LAVA-Flex and IDEAL-IQ techniques in patients with metabolic dysfunction-associated fatty liver disease(MAFLD).To explore the feasibility of the LAVA-Flex technique for rapid detection of whole-liver fat quantification.Methods:97 patients with MAFLD and 23 healthy volunteers were enrolled.IDEAL-IQ sequence scan was performed to obtain the PDFF,R2*,water-phase,fat-phase,in-phase and opposed-phase maps,and LAVA-Flex sequence scan was performed to obtain the water-phase,fat-phase,in-phase and opposed-phase maps.3D Slicer software was used to outline the whole liver in the PDFF,water-phase and fat-phase maps,and the fat signal fractions(FFIQ and FFFlex)of the whole liver in the respective water-phase and fat-phase maps were calculated.According to the PDFF,120 cases were divided into 4 groups,the normal group(23 cases,PDFF<5%),the mild group(15 cases,5%≤PDFF<10%),the moderate group(48 cases,10%≤PDFF<25%)and the severe group(34 cases,PDFF≥25%).Correlation analysis was used to compare the consistency between FFIQ,FFFlex and PDFF in 4 groups,and correlation heat maps and Bland-Altman plots were drawn.Results:There was a significant correlation between the total FFFlex and PDFF(r=0.959).In the normal group,there was a significant correlation between FFIQ and PDFF(r=0.991,P<0.01).In the mild group,there was a relatively strong and significant correlation between FFIQ and PDFF(r=0.848,P<0.01).In the moderate group,the correlation between FFIQ and PDFF was extremely strong and significant(r=0.997,P<0.01),and the correlation between FFFlex and PDFF was relatively strong and signifi-cant(r=0.756,P<0.01).In the severe group,both FFIQ and FFFlex correlated significantly with PDFF(r=0.779,0.793,both P<0.01).The Bland-Altman plots showed that most of the data points of FFFlex and FFIQ fell within the limits of agreement,and the center line was close to 0,indicating their good consistencies with PDFF.Conclusions:Both the IDEAL-IQ and LAVA-Flex techniques show a high degree of consistency with PDFF in measuring the whole-liver FF,especially in patients with moderate and severe fatty liver.The LAVA-Flex technique,with the advantage of rapid imaging,provides another powerful tool for the determination of liver fat content in clinical practice.
