钆塞酸二钠增强MRI对肝硬化患者肝功能的量化评估
Quantitative evaluation of liver function in patients with liver cirrhosis based on Gd-EOB-DTPA enhanced MRI

Abstract
Objective:To explore the evaluation of imaging indicators on liver function in patients with liver cirrhosis by Gd-EOB-DTPA enhanced MRI.Methods:The clinical and imaging data of 76 patients with liver cirrhosis were retrospectively analyzed.The Child-Pugh classification,score for end-stage liver disease(MELD)model,indocyanine green retention rate at 15 minutes(ICG-R15),albumin-bilirubin(ALBI)score were evaluated,and the liver-to-spleen ratio(LSR),liver-to-muscle ratio(LMR),relative enhancement index(REI),postcontrast T1(T1post),the reduction rate of T1(rrT1),and the functional liver imaging score(FLIS)were calculated.Based on Child-Pugh classification,MELD score,ICG-R15,ALBI score and using k-means clustering algorithm,76 patients were divided into the good function group(52 cases)and the poor function group(24 cases).The imaging parameters in two groups were compared and ROC curve analysis was performed to determine the optimal critical value.Results:There were statistically significant differences in LMR,LSR,FLIS,T1post and rrT1 not only among different subgroups based on Child-Pugh classification,MELD score,ICG-R15,ALBI score,but also between the good function group and the poor function group(all P<0.05).The critical values for evaluating liver dysfunction in LMR,LSR,FLIS,T1post and rrT1 were 2.65,1.76,5.00,347.67 ms and 61.16%,and the AUC values were 0.916,0.906,0.902,0.951 and 0.909,respectively.LMR had the highest sensitivity and rrT1 had the highest specificity.Conclusions:Gd-EOB-DTPA enhanced MRI parameters(LMR,LSR,FLIS,T1post and rrT1)can be used as the effective diagnostic tools for evaluating liver function in patients with liver cirrhosis.关键词
