Value of CT-measured extracellular volume fraction(EVF)in predicting pattern of vessel encapsulated tumour cluster(VETC)in hepatocellular carcinoma

Abstract
Objective:To investigate the value of CT-measured extracellular volume fraction(ECV)in predicting the pattern of vessel encapsulated tumour cluster(VETC)in patients with hepatocellular carcinoma(HCC).Methods:A total of 101 HCC patients,confirmed by surgical pathology,were included in the study.Based on the immunohistochemical results,101 patients were divided into a VETC-positive group(46 cases)and a VETC-negative group(55 cases).Preoperative multi-phase dynamic CT enhancement scans were performed on all patients.The CT values of the abdominal aorta and tumors were measured on both plain phase and enhancement equilibrium phase,then ECV was calculated.The imaging features and clinical data of the two groups were compared,binary logistic regression was used to identify independent risk factors for positive VETC,and a nomogram model was constructed.The diagnostic performance of the independent risk factors and the combined model was evaluated using ROC curve analysis.Results:Multivariate logistic regression analysis identified AFP≥400 μg/L,intratumoral artery and ECV as independent risk factors for positive VETC(OR=2.695,3.633,1.061,all P<0.05).The ROC curve showed that the AUCs for AFP≥400 μg/L,intratumoral artery,ECV and the nomogram model in predicting the positive VETC were 0.617,0.679,0.662 and 0.781,respectively.The nomogram model demonstrated superior diagnostic performance compared to each individual predictor,with statistically significant differences(Z=3.490,2.759,2.330,all P<0.05).Conclusions:The nomogram model based on AFP≥400 μg/L,intratumoral artery and CT-measured ECV is valuable for preoperatively predicting the VETC pattern in HCC and has potential clinical implications.关键词
