耳甲电针治疗早中期帕金森病即刻脑效应的fMRI研究
An fMRI study of immediate effect of transcutaneous auricular vagus nerve stimulation in treatment of early to mid-stage Parkinson's disease

Abstract
Objective:To observe the immediate regulatory effect of electroacupuncture on the auricular nail,namely transcutaneous auricular vagus nerve stimulation(taVNS),on the resting-state fMRI brain functional activities in early to mid-stage Parkinson's disease patients.Methods:Seventeen patients with early to mid-stage Parkinson's disease who took regular medication were selected as the case group,and 17 healthy controls matched with gender,age and education years were selected as the control group.The case group was treated with taVNS for 30 minutes,and resting-state fMRI scans were performed before and after treatment.Only resting-state fMRI scan was performed in the control group.The differences of brain-wide amplitude of low-frequency fluctuations(ALFF),fractional ALFF(fALFF)and regional homogeneity(ReHo)between the two groups were observed and compared,as well as the changes before and after taVNS.Results:Compared with the control group,the cerebella ALFF values,the fALFF values in the right cuneiform lobe/right talar sulcus,left precentral gyrus/left postcentral gyrus,and ReHo values in the right cerebellum,right rectangular sulcus/right cuneiform lobe/right lingual gyrus in the case group were reduced.After immediate taVNS,the ALFF values of bilateral supplementary motor area(SMA)/bilateral superior frontal gyri/left anterior cingulate/left paracingulate gyrus were increased in the case group compared with before taVNS.The above differences were statistical(GRF correction,cluster P<0.05,threshold voxel P<0.005).Conclusions:Patients with early to mid-stage Parkinson's disease who take regular medication still have abnormal brain function in cerebella and sensory and motor cortical brain regions,and the immediate modulation effect of taVNS is mainly concentrated in bilateral SMA and prefrontal lobes,which may be the brain mechanism of taVNS in the adjuvant treatment of patients with early to mid-stage Parkinson's disease.关键词
