Abstract

Objective:To explore the application of conventional ultrasound,shear wave elastography(SWE)and contrast-enhanced ultrasound(CEUS)in the diagnosis of non-mass breast lesions,and the correlation between CEUS features and different molecular types of invasive non-mass breast cancer,in order to improve the diagnostic efficiency of non-mass breast lesions.Methods:A total of 158 patients with non-mass breast lesions were collected.The conventional ultrasound,SWE and CEUS were used for the diagnosis,and the features of benign and malignant lesions were analyzed.ROC curve was used to evaluate the diagnostic efficacy.Spearman rank correlation was used to analyze the correlation between the CEUS characteristics and tumor molecular typing of the invasive non-mass breast cancer.Results:Among 158 cases,there were 95 malignant lesions and 63 benign lesions.The benign lesions were characterized by synchronous/slow progression of contrast medium,uniform low/equal enhancement,and an unexpanded enhancement range.The malignant lesions were characterized by rapid entry of contrast medium,uneven and high enhancement,and an expanded enhancement range.There were statistically significant differences between benign and malignant lesions(all P<0.05).The diagnostic effeciency of multimodal ultrasound for benign and malignant lesions was the highest,with an AUC of 0.93,a sensitivity of 85.26%,a specificity of 93.65%.In 80 patients with invasive cancer,compared with luminal B type lesions,luminal A type lesions were more likely characterized by an expanded enhancement range and peripheral vascular sign,the differences were statistical(both P<0.05).The HER-2-overexpression type and Basal-like type lesions showed an unexpanded enhancement range and unclear edges after enhancement(both P<0.05).Conclusions:Multimodal ultrasound can improve the diagnostic efficiency of non-mass breast lesions.The characteristics of CEUS are closely related to the molecular typing of invasive non-mass breast cancer.
