盆底实时四维超声联合剪切波弹性成像对自然分娩初产妇压力性尿失禁的评估价值
Evaluation of pelvic floor real-time four-dimensional ultrasound combined with shear wave elastography for postpartum stress urinary incontinence in primipara with natural childbirth

Abstract
Objective:To explore the value of pelvic floor real-time four-dimensional ultrasound combined with shear wave elastography(SWE)in evaluating postpartum stress urinary incontinence(SUI)in primipara with natural childbirth.Methods:A total of 130 primiparas who gave birth naturally were included.According to whether SUI occurred 42 days after delivery,they were divided into SUI group(46 cases)and non-SUI group(84 cases).Both groups underwent pelvic floor real-time four-dimensional ultrasound combined with SWE,and the relevant parameters were obtained.The ROC curve was used to analyze the value of pelvic floor real-time four-dimensional ultrasound combined with SWE in evaluating postpartum SUI.Results:The posterior urethral angle,bladder neck mobility and levator hiatus area(LHA)under Valsalva maneuver and the proportion of funnel formation in the urethra in the SUI group were larger than those in the non-SUI group(all P<0.05).The mean value of Young's modulus(Emean)of the right puborectalis in resting-state and the Emean of bilateral puborectalis under Valsalva maneuver in SUI group were significantly lower than those in the non-SUI group(both P<0.05).ROC curve showed that the AUCs of posterior vesicourethral angle,bladder neck mobility,LHA,bilateral puborectalis Emean under Valsalva maneuver and the right puborectalis Emean in resting-state to evaluate postpartum SUI were>0.7,while the AUC of the above combined parameters to evaluate postpartum SUI was>0.85,with a specificity of 0.833 and a sensitivity of 0.826,respectively,and the maximum Youden index was 0.659.Conclusions:Pelvic floor real-time four-dimensional ultrasound combined with SWE has a high value in evaluating postpartum SUI in primiparas with natural childbirth,can greatly improve the accuracy of clinical evaluation of SUI.关键词
