Efficacy of Doxycycline Combined with Hysteroscopic Polypectomy and Endometrial Scratching in the Treatment of Repeated Implantation Failure Complicated with Chronic Endometritis

Abstract
Objective:To investigate the clinical efficacy of doxycycline combined with hysteroscopic polypectomy and endometrial scratching in the treatment of repeated implantation failure(RIF)complicated with chronic endometritis(CE).Methods:A total of 162 patients with RIF complicated with CE treated in the Second Affiliated Hospital of Zhengzhou University from February 1,2023 to August 31,2024 were selected and assigned to the control group and the observation group by random number table method,with 81 patients in each group.The control group was treated with hysteroscopic polypectomy and endometrial scratching,while the observation group was treated with doxycycline hyclate tablets in addition to the treatment given in the control group for 14 consecutive days.The number of CD38-positive and CD138-positive cases,inflammatory factors[tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),interleukin-8(IL-8)],endometrial thickness on the day of endometrial transformation,subsequent embryo transfer(number of transferred embryos,number of high-quality embryos transferred)and pregnancy follow-up(clinical pregnancy rate,early miscarriage rate,live birth rate)were compared between the two groups.Results:After treatment,the number of CD38-positive and CD138-positive cases and levels of TNF-α,IFN-γ and IL-8 were decreased in both groups(P<0.05),with the observation group showing lower levels than the control group(P<0.05).No statistically significant differences in the endometrial thickness on the day of endometrial transformation,number of transferred embryos and number of high-quality embryos transferred were observed between the two groups(P>0.05).The clinical pregnancy rate in the observation group(61.04%)was higher than that in the control group(44.74%,P<0.05).The early miscarriage rate in the observation group(4.26%)was lower than that in the control group(17.65%,P<0.05).The live birth rate in the observation group(76.60%)was higher than that in the control group(52.94%,P<0.05).Conclusion:Doxycycline combined with hysteroscopic polypectomy and endometrial scratching can promote the inflammatory resolution in patients with RIF complicated with CE,and improve the success rate of pregnancy in patients with embryo transfer.关键词
