口腔颌面肿瘤手术患者术后感染病原学特征、耐药性及危险因素分析
Analysis of Pathogenic Characteristics,Drug Resistance,and Risk Factors of Postoperative Infection in Cases with Oral and Maxillofacial Tumors

Abstract
Objective To analyze the pathogenic characteristics,drug resistance,and risk factors of postoperative infection in cases with oral and maxillofacial tumors.Methods A total of 85 patients with oral and maxillofacial tumors diagnosed and treated surgically at the Affiliated Hospital of Yan'an University from January 2022 to January 2024 were selected.Clinical data were collected,and the risk of postoperative infection,distribution and drug resistance of pathogens,and risk factors for postoperative infection were analyzed using multivariate Logistic regression.Results Postoperative infection occurred in 23 of 85 patients(27.06%).The main infection sites were surgical incisions(43.48%)and the respiratory tract(34.78%).A total of 45 non-repetitive strains were isolated,including 24 gram-positive bacteria(53.33%),19 gram-negative bacteria(42.22%),and 2 fungi(4.44%).Staphylococcus aureus showed a resistance rate of 90.91% to penicillin but no resistance to linezolid or vancomycin.Pseudomonas aeruginosa showed a resistance rate of 83.33% to ampicillin but no resistance to imipenem,vancomycin,or aztreonam.Diabetes history,intraoperative antibiotic use,operative time ≥3 h,and postoperative albumin<35 g/L were identified as risk factors for postoperative infection(P<0.05).Conclusion Gram-positive bacteria are the main pathogens of postoperative infection in oral and maxillofacial tumors,with significant antibiotic resistance.Rational use of antibiotics and targeted prevention and control measures should be implemented to reduce postoperative infection rates.关键词
