Abstract
Objective To explore the expression levels of serum CXC chemokine ligand 8(CXCL8)and a desintegrin and metalloproteinase 17(ADAM17)in patients with chronic spontaneous urticaria(CSU)and their clinical significance.Methods A total of 77 CSU patients treated in Zigong First People's Hospital from January 2023 to September 2023 were selected as the study group,and 50 healthy subjects receiving physical examination in the same hospital during the same period were selected as the control group.The basic data of the patients were collected,and the serum CXCL8 and ADAM17 levels were detected and compared by enzyme-linked immunosorbent assay(ELISA).According to 7-day urticaria activity score(UAS7),patients in the study group were divided into three subgroups:mild CSU group(0-14 points,19 cases),moderate CSU group(15-29 points,32 cases)and severe CSU group(30-42 points,26 cases).Serum CXCL8 and ADAM17 levels were compared between the three subgroups.The correlation of serum CXCL8 and ADAM17 levels with UAS7 in CSU patients was analyzed by Pearson correlation,and the predictive value of serum CXCL8 and ADAM 17 levels on the severity of CSU were analyzed by receiver operating characteristic(ROC)curves.Results Serum CXCL8 and ADAM17 in the study group were significantly higher compared with those of the control group(P<0.05).With the increase of the severity,serum CXCL8 and ADAM17 in CSU patients increased successively(severe CSU group>moderate CSU group>mild CSU group)(P<0.05).Serum CXCL8 and ADAM17 levels were positively correlated with UAS7 in CSU patients(r=0.619,and 0.563,respectively;P<0.001).ROC curve analysis showed that the area under the curve(AUC)of serum CXCL8 combined with ADAM17 in predicting moderate CSU was 0.913,which was larger than the single indicator(serum CXCL8:0.841;serum ADAM17:0.769;Z=8.142,and 7.565,respectively;P<0.001).The AUC of serum CXCL8 combined with ADAM 17 in predicting severe CSU was 0.910,which was larger than the single indicator(serum CXCL8:0.849;serum ADAM17:0.775;Z=6.718,and 5.826,respectively;P<0.001).Conclusion Serum CXCL8 and ADAM 17 levels are highly expressed in CSU patients,and the expression levels are related to the severity of CSU.The combined detection of serum CXCL8 and ADAM 17 levels can predict the severity of CSU.关键词
