摘要
Abstract
Objective To explore the clinical effect of recombinant human bone morphogenetic protein-2(rhBMP-2)combined with platelet-rich fibrin(PRF)in preserving tooth extraction sites in bone wall defects.Methods A total of 196 patients who underwent tooth extraction site preservation for bone wall defects in the Yuncheng Central Hospital Affiliated to Shanxi Medical Universityfrom March 2021 to March 2024 were retrospectively collected.They were divided into the control group(n=63,PRF implantation),observation group(n=69,implantation of rhBMP-2 and PRF),and traditional group(n=64,implantation of collagen sponge)according to different tooth extraction socket filling materials.Alveolus-related evaluation indicators(cone beam computed tomography[CBCT]indicators,alveolar crest bone defects,gingival recession)and wound healing indicators(wound healing rate,wound healing appearance and infection assessment[spontaneous bleeding,persistent swelling,ulcer],and pain level during wound healing)were compared among the three groups.Results At 18 weeksafter operation,the CBCT indicators of the observation group were superior to those of the control group and the traditional group,and the CBCT indicators of the control group were superior to those of the traditional group(P<0.05).The number of cases of alveolar crest bone defects at 18 weeks after operation was significantly less in the observation group than the control group and the traditional group(P<0.05),and without significant differences between the control group and the traditional group(P>0.05).There were no significant differences between the control group and the observation group in the gingival recession values of mesial gingival papilla and distal gingival papilla(P>0.05),but significantly less than those of the traditional group(P<0.05).The gingival recession values at midpoint of labial gingival margin in the observation group were significantly less than those of the control group and the traditional group,which were significantly less in the control group than the traditional group(P<0.05).At 7days after operation,the wound healing rate in the observation group was significantly higher than that of the control group and the traditional group,which was significantly higher in the control group than the traditional group(P<0.05).There were no significant differences in total incidence of spontaneous bleeding,persistent swelling and ulcer between the control group and the observation group(P>0.05),but significantly less than those of the traditional group(P<0.05).The wound healing time of the observation group was significantly shorter than that of the control group and the traditional group(P<0.05),and the control group was significantly shorter than the traditional group(P<0.05).The Visual Analogue Scale(VAS)score of wound healing at 1 day and 7 days was significantly lower in the observation group than the control group and the traditional group,which was significantly lower in the control group than the traditional group(P<0.05).Conclusion rhBMP-2 combined with PRF has a good effect on the preservation of tooth extraction sites with bone wall defects.It can effectively promote tooth extraction wound healing,reduce alveolar bone absorption,and prevent gum recession.关键词
