Clinical Value of Combined Diagnosis of Hip Joint Stability in Children with Developmental Dysplasia of the Hip Based on Ultrasound Quantitative Examination Results

Abstract

Objective To assess the value of ultrasound quantitative parameters in diagnosing children with developmental dysplasia of the hip(DDH),thus providing references for early diagnosis and formulation of treatment regimens to improve the prognosis.Methods From June 2022 to June 2024,68 children with DDH in the RuGao Women's and Children's Health and Family Planning Service Center were selected.All were examined with hip adduction stability test.According to the test results,children were divided into hestable group(n=40)and unstable group(n=28).Quantitative ultrasound examination was conducted to compare the differences in alpha angle,beta angle,head to pubic distance,femoral head diameter,and femoral head coverage rate between groups.The correlation among alpha angle,beta angle,head to pubic distance,femoral head diameter,and femoral head coverage rate was analyzed.Multivariate logistic regression analysis was conducted to investigate the influencing factors for hip joint stability in children with DDH.The receiver operating characteristic(ROC)analysis was used to predict the clinical value of ultrasound quantitative parameters in evaluating hip joint stability in children with DDH.The occurrence of ossification centers in DDH children of different ages was analyzed.Results Based on the Graf typing among the 68 patients,there were 14 cases of type Ⅱa,28 cases of type Ⅱb,14 cases of type Ⅱc,10 cases of type D,and 2 cases of typeⅢ.The alpha angle,femoral head coverage,and femoral head diameter were significantly lower in the unstable group than the stable group,while the beta angle and head to pubic distance were significantly higher(P<0.05).The alpha angle was positively correlated with femoral head coverage and diameter,and negatively correlated with head to pubic distance(P<0.05).The beta angle was negatively correlated with femoral head coverage and diameter,and positively correlated with head to pubic distance(P<0.05).Multivariate logistic regression analysis showed that alpha angle,beta angle,head to pubic distance,femoral head diameter,and femoral head coverage were independently correlated with hip joint stability in children with DDH.ROC analysis showed that the area under the curve(AUC)of a combined detection of alpha angle,beta angle,head to pubic distance,femoral head diameter,and femoral head coverage in predicting the prognosis of DDH was 0.926,with a sensitivity of 90.75%,and a specificity of 80.57%,which was higher than the single detection.Out of 68 children with DDH,10 had ossification centers,and there was a statistically significant difference in the incidence of ossification centers at different ages(P<0.05).Conclusion Ultrasound quantitative parameters can evaluate the stability of the hip joint in children with DDH,improve the accuracy and reliability of diagnosis,and help detect hip joint instability at an early stage,providing strong evidence for timely and accurate treatment intervention in the future.It is worth further promoting and applying in clinical practice.
