Impact of the interaction between blood uric acid and dyslipidemia on type 2 diabetes peripheral neuropathy

Abstract
Objective To explore the impact of the interaction between blood uric acid and dyslipidemia on type 2 diabetes peripheral neuropathy(T2DPN),and to provide a reference for clinical diagnosis and treatment of T2DPN.Methods A total of 152 patients with type 2 diabetes mellitus(T2DM)were selected from the Department of Endocrinology,Maanshan General Hospital of Ranger-Duree Healthcare from June 2022 to June 2024,of which 32 diagnosed with simple T2DM were included in the T2DM group and 120 with T2DPN were included in the T2DPN group.The abnormal rates of blood uric acid and blood lipids(triglyceride[TG],total cholesterol[TC],high-density lipoprotein cholesterol[HDL-C],low-density lipoprotein cholesterol[LDL-C])and nerve conduction velocity(median motor nerve conduction velocity[MMCV],peroneal motor nerve conduction velocity[PMCV])were compared between the T2DM group and the T2DPN group.According to the vibration perception threshold(VPT),patients were divided into mild(VPT 16-24 V,44 cases),moderate(VPT 25-39 V,40 cases),and severe subgroups(VPT ≥40 V,36 cases)in the T2DPN group.The blood uric acid,dyslipidemia rate,and nerve conduction velocity of different disease severity levels were compared in the T2DPN group.The correlation between blood uric acid,dyslipidemia,nerve conduction velocity,and the severity of T2DPN was analyzed.The impact of blood uric acid and dyslipidemia on T2DPN,as well as the impact of their interaction on T2DPN was analyzed.The predictive effect of blood uric acid,dyslipidemia,and their interaction on T2DPN was analyzed.Results The levels of blood uric acid,TG,TC,LDL-C,and the prevalence of dyslipidemia in the T2DPN group were significantly higher than those of the T2DM group,while the levels of HDL-C,MMVC,and PMVC were significantly lower than those of the T2DM group(P<0.05).The levels of blood uric acid,TG,and dyslipidemia in the mild subgroup were significantly lower than those of the moderate and severe subgroups,which were significantly lower in the moderate subgroup than the severe subgroup.The levels of HDL-C,MMVC,and PMVC in the mild subgroup were the highest,followed by the moderate subgroupandthe severe subgroup(P<0.05).The levels of blood uric acid and dyslipidemia were negatively correlated with both MMCV and PMCV,and positively correlated with the severity of T2DPN.There was a positive correlation between blood uric acid and dyslipidemia(P<0.05).Both blood uric acid and dyslipidemia were independent factors for the development of T2DPN(P<0.05).High blood uric acid and dyslipidemia showed a positive interaction in T2DPN.The area under the curve(AUC)of an interaction of high blood uric acid levels and dyslipidemia in predicting T2DPN was 0.876(95%CI:0.813-0.924),which was greater than that of high blood uric acid levels and dyslipidemia alone(P<0.05).Conclusion High blood uric acid and dyslipidemia are independent factors for the development of T2DPN,and there is a positive interaction between them.The interaction between high blood uric acid and dyslipidemia has a high predictive efficacy for T2DPN.关键词
