急性AT型脑卒中患者血清Lp-PLA2、hs-CRP及NLR水平的变化及临床意义
Changes and Clinical Significance of Serum Lp-PLA2,hs-CRP and NLR levels in Patients with Acute AT-type Stroke

Abstract
Objective:To investigate the changes of serum lipoprotein phospholipase A2(Lp-PLA2),high-sensitivity C-reactive protein(hs-CRP),and neutrophil-to-lymphocyte ratio(NLR)in patients with acute atherothrombotic(AT)-type stroke,and to evaluate their clinical significance.Method:A total of 100 patients with acute AT-type stroke who admitted to Foshan Fosun Chancheng Hospital from January 2022 to December 2023 were selected as the stroke group,and they were divided into mild group(<4 scores,n=50),moderate group(4-15 scores,n=25)and severe group(>15 scores,n=25)according to the National Institutes of Health stroke scale(NIHSS)score;fifty-five healthy subjects were selected as the control group.All patients were followed up for 30 d,according to the modified Rankin scale(mRS)score,the stroke group was divided into the good prognosis group(mRS score<2 scores,67 cases)and the poor prognosis group(mRS score≥2 scores,33 cases).The levels of serum Lp-PLA2,hs-CRP and NLR in stroke group and control group,mild group,moderate group and severe group,good prognosis group and poor prognosis group were compared,and the diagnostic efficacy of single and combined detection of Lp-PLA2,hs-CRP and NLR in acute AT-type stroke was evaluated.Result:The levels of serum Lp-PLA2,hs-CRP and NLR in the stroke group were higher than those in the control group,and the differences were statistically significant(P<0.05);the levels of serum Lp-PLA2,hs-CRP and NLR in the severe group were higher than those in the moderate group,and those in the moderate group were higher than those in the mild group,the differences were statistically significant(P<0.05);the area under the receiver operating characteristic curve(AUC)of combined diagnosis was better than those of Lp-PLA2,hs-CRP and NLR alone,and the differences were statistically significant(P<0.001);the levels of serum Lp-PLA2,hs-CRP and NLR in the good prognosis group were lower than those in the poor prognosis group,and the differences were statistically significant(P<0.05).Conclusion:The levels of serum Lp-PLA2,hs-CRP and NLR in patients with acute AT-type stroke are increased,and the combined detection shows high diagnostic value,which is closely related to the degree of early neurological impairment and the poor prognosis of patients.关键词
