Abstract
Objective:To investigate the predictive value of in-stent restenosis(ISR)after stenting in patients with ischemic stroke(CIS).Method:A total of 115 CIS patients who underwent carotid artery stenting at Jingmen Traditional Chinese Medicine Hospital from January 2021 to April 2023 were selected as the study objects.Clinical data of the patients were collected and lipid metabolism indexes were detected.After 1 year of follow-up,patients were divided into ISR group(n=30)and non-ISR group(n=85)according to whether they had ISR.The risk factors of ISR after carotid stenting in CIS patients were analyzed,and the predictive value of lipid metabolism indexes on ISR after carotid stenting in CIS patients was analyzed.Result:The number of stents and the degree of residual stenosis in ISR group were higher than those in non-ISR group(P<0.05).There was no significant difference in the comparison of gender,smoking history,drinking history,hypertension,diabetes mellitus,coronary heart disease,age,body mass index and preoperative National Institutes of Health Stroke Scale(NIHSS)score between the two groups(P>0.05).Lipid metabolism indexes,Total Cholesterol(TC),Triglyceride(TG)and Low-Density Lipoprotein(LDL-C)in ISR group were higher than those in non-ISR group,and HDL-C level was lower than that in non-ISR group(P<0.05).Residual stenosis(OR=4.332),TC(OR=5.265),TG(OR=6.160),HDL-C(OR=4.963),LDL-C(OR=3.789)were risk factors for ISR after carotid stenting in CIS patients.ROC curve analysis showed that the combined detection of TC,TG,HDL-C and LDL-C predicted the AUC(95%CI)of ISR after carotid stents in CIS patients was 0.940(0.877~0.997),the sensitivity was 91.7%,and the specificity was 97.8%.All of which were higher than the independent detection of each index.Conclusion:The lipid metabolism indexes TC,TG,HDL-C and LDL-C in CIS patients are risk factors affecting the occurrence of ISR after carotid stents in CIS patients,and the combined detection of all indexes can improve the predictive value of ISR.关键词
